ARE YOU A VETERAN – of any war - WHO HAS HAD MEDICAL PROBLEMS WHILE VISITING UNITED STATES OF AMERICA?

ARE YOU A VETERAN – of any war - WHO HAS HAD MEDICAL PROBLEMS WHILE VISITING AUSTRALIA?

We are seeking information on the problems you have experienced.

We want to know your name, rank and a short list of war related medical problems.

We want to know the difficulty you had in obtaining help for those war related problems while visiting either Australia or America.

CAN YOU HELP US TO HELP YOU BY SUPPLYING THIS INFORMATION?

If you are an Australian who has visited U.S.A. did you

1. Contact Canadian Embassy for assistance while in USA?
2. Did you contact a local V.A. Hospital?
3. Did you produce your gold card?
4. If so, what happened?
5. Did you pay for your own medical fees?
6. Were you re-imbursed on arrival back in Australia?
7. Did you take out private health insurance while overseas?
8. What did DVA advise you before you left Australia?

Any other information would be valuable.
